Information Criteria
RADIOACTIVE
- Waste/material physical location:
- Description of waste/material:
(include isotopes, activities, form and immediate container)
- Nitrates/Acetate, grams of material and weight of primary container in grams
- Dimensions of sources/sealed sources (if applicable):
- Date sources were manufactured (if applicable):
- Leak test results (if applicable):
- Export Petitions/Disposal Permits (if applicable)
HAZARDOUS
- Names of all of the chemicals involved:
- Final concentrations of all of the chemicals involved (must total 100%)
- State of the waste (i.e. liquid, solid, gas, slurry, sludge, etc.)
- Any available analytical data (e.g. pH, flashpoint, TCLP, MSDS, etc.)
- Complete description of the immediate container (e.g. size, type, condition,
specification, etc.)
